I'm seeking a way to group question types in a response form container wrapper, ideally a Fieldset, or alternatively a Div. I spent 2 days playing around with default-form.php but now need to ask the experts.

The need is to be able to make a form where 3 questions are grouped so as to be seen as a single choice to vote and comment on. The attached image hopefully explains it better. There will eventually be around 10 choices where I want each choice to be seen as a row., all on a single response page.
