I have watched the video on youtube but still I am not quite sure, if this survey extension is what I need.

We will publish the survey in our magazine and also want to provide the opportunity to do the survey online on the magazines webpage. Among all the people who do the survey, we will draw a reward. So I need to get the name and adress of everyone who partakes of it. In the magazine that is easy. They answer the questions and then fill in the adress data before cutting the page out and sendig it to us. But how do I get the names and adresses of everybody who fills in the survey online?
