I am unable to complete a survey

I know I have not completed a survey and yet when I login it tells me that I already have. The survey is restricted to username only, so I know I have created a new user who has never completed the survey before. The web site is www.acalltoaction.org.uk

I get this message below when I login in. It is Joomla 3.4.0
