Hi,

When i add a survey as a module or a menu link the page does not work, appears as a 404 - Page not found.

Joomla! 3.3.6 Stable

PHP version 5.4.45

In fact when i check now all pages displaying any survey, forms or results are all appearing as 404.

Example:

http://spm.sch.id/index.php/our-school/jr-sr-high/an-amazing-study-opportunity

Please help.
