Unable to get captcha to show up on https://amacus.net/survey

I have set in the default config for the captcha and set up the plugin which is enabled. I have also activated in the community survey options and unable to get it to show still. Can you please let me know what I am doing wrong.

I have survey intro turned on I believe as well
