We are creating a survey which we use the "Radio Grid" option,  Please see my 2 examples below.  I would like to add what each number represnts on top of the columns 1 = unimportant : 5= very important/ 1=never : 5 = always.  It it is only allowing me to add the columns answers and that's all. 

Also example 1 has a dual grid.  Is this possible to do.  If not the second example would be ok as well if we can just the rating above it.
