I'm having a really impossible time using the surveys component I purchased.  I'm using RocketTheme's Salient template and your form items are not showing via the front end in a way that makes it possible to use.   Likewise, the survey options are not showing in the backend at all.  Lastly, the documentation is completely out of data which adds to the difficulty.
