If you want to disable the toolbar, disable "Show Toolbar" option in the component options. If you want to change it, you need to edit the layout file... -

Yes, the community quiz does not have its own points system. -

The numbers are mile markers.. starts at 1 and continue.. the milestone distance can vary. I will change the text to show distance instead of mile number. -

I do not see any issue with the package, it installs fine on several tests. Is your PHP max file upload limit is less than 6MB? -

Create Category menu item for your category and publish the module on that menu item. You need not publish menu item on front-end, you can create it in a new menus. -

There are integrations for third-party components. Select the component which you are using. If you are not using any points system component, you can leave it unselected. -

Responses cannot be edited. If you want to edit the data manually, you can do so from your MySQL client in the table xxx_survey_response_details. Search with response id and... -

I am afraid that the code does not provide the necessary information to debug the issue. Can you post full error details from your apache log file?
