Were you using any other quiz component before installing Community Quiz? If yes, rename the existing tables that start with xxx_quiz_ and install the component again. -

The plugin is part of the ACY Mailing component itself. Go to your ACYMailing component and install it from addons page. It is available only for ACYMailing v6 or later. -
